KRAS and BRAF mutation screening in metastatic colorectal cancer ...

Researchers report that screening for KRAS and BRAF mutations can reduce the cost of anti-EGFR treatment for metastatic colorectal cancer but with a very small reduction in overall survival according to a new study published on November 28 in the Journal of the National Cancer Institute.

Metastatic colorectal cancer patients whose tumors harbor mutations in KRAS (and to a lesser extent, in BRAF) are unlikely to respond to costly anti-EGFR therapies. Screening of patients who are candidates for these therapies for mutations in one of these genes (KRAS) has been recommended, with the goal of providing treatment to those who are likely to benefit from it while avoiding unnecessary costs and harm to those who are not likely to benefit. However, the real-world impact of mutation screening for both KRAS and BRAF is unclear.

To better understand the impact of mutation screening with regard to health outcomes, costs, and value, Ajay S. Behl, Ph.D., M.B.A., of the HealthPartners Research Foundation in Bloomington, Minnesota, and colleagues, performed a cost-effectiveness analysis that took into account the treatments, resection of metastases, and survival for the different types of metastases. They conducted patient-level decision analytic simulation modeling comparing four strategies involving KRAS and BRAF mutation testing to select treatments for metastatic colorectal cancer patients: no anti-EGFR therapy (best supportive care); anti-EGFR therapy without screening; screening for KRAS mutations only (before providing anti-EGFR therapy); and screening for KRAS and BRAF mutations (before providing anti-EGFR therapy).

The researchers found that compared with no anti-EGFR therapy, screening for both KRAS and BRAF mutations showed a very high (ie, unfavorable) incremental cost-effectiveness ratio, meaning it was very costly in relation to its benefits. Compared with anti-EGFR therapy without screening, screening for KRAS mutations saved approximately $7,500 per patient; adding BRAF mutation screening saved another $1023, with little reduction in expected survival.

The authors write, "In general, our results are less supportive of the use of anti-EGFR therapy than previous analyses, and they indicate lower cost savings from KRAS testing than previously reported. Although we cannot confirm that anti-EGFR therapy is a cost-effective use of health care resources, we can affirm that KRAS testing is cost-saving. BRAF testing may offer additional savings."

In an accompanying editorial, Josh J. Carlson, M.P.H., Ph.D., of the Department of Pharmacy, University of Washington, and Scott D. Ramsey, MD, PhD, of the Division of Public Health Sciences, Fred Hutchinson Cancer Research Center, both in Seattle, note two practical points highlighted by the study: that molecular testing is as much about generating cost savings by identifying nonresponders as it is about improving survival by identifying responders, and that good modeling must account for the fact that community practice (as opposed to clinical trials) "is messy." They write, "most importantly, this study of an unusually accurate test raises important issues that should be considered for other molecular tests in other settings."

Turkish Airlines eyes possible $500 million lease deal: bankers

ISTANBUL (Reuters) - Turkey's national carrier Turkish Airlines is in talks with banks about a leasing arrangement worth around $500 million to help fund its plane orders from 2014, bankers familiar with the negotiations said on Friday.

The airline is considering using Enhanced Equipment Trust Certificates (EETCs) for the first time - financial securities issued by banks under which the airline gets ownership of the planes when the certificates mature, the bankers said.

Turkish Airlines was also considering issuing sukuk - Islamic bonds - or eurobonds next year to finance its aircraft acquisition plans from 2014, they said.

Officials at the airline could not be reached for comment.

"Turkish Airlines is seeking various financing opportunities abroad. Sukuk or eurobond issuances might be also on the agenda, but EETC is outweighing them, which would be used for the first time for financing aircraft orders," said one banker.

"Turkish Airlines' borrowing would not be small in size, around at least $300-$500 million for each transaction."

Turkish Airlines said last month it had decided to buy 15 Boeing B777-300ERs by 2017, with an option to buy five more, and 15 Airbus A330-300 planes between 2014-2016.

EETC transactions, effectively a form of secured debt financing like mortgages, are often used by airlines in Europe and the United States, but are rarer elsewhere.

Aircraft leasing firm Doric Nimrod Air Finance Alpha said in June it would use EETCs worth almost $590 million to help finance the sale of four Airbus A380 planes to Emirates airline.

Reliance Life hires 200 women insurance advisors | Firstpost

Mumbai: Reliance Life Insurance Company (RLIC) today said it has hired around 200 women insurance advisors for its distribution channel and plans to scale up the recruitment in the next few months.

This was announced by the company while launching its ?Face-to-Face? distribution channel is aimed at servicing customers whose agents have become inactive. ?Reliance Life Insurance has hired around 200 women employee advisors in seven cities during the pilot phase, and plans to scale up across the country in the next few months,? the company said in a statement.

RLIC, part of Anil Ambani-led Reliance Group?s financial services arm Reliance Capital, said ?Face-to-Face? distribution channel would service its orphan customers and provide post-sales service to policyholders.

In such cases, the responsibility of servicing these policies is transferred to an active agent, who stands to earn commission on the renewal premiums.

The company said the new distribution channel would operate with Life Planning Officers (LPOs) who would primarily be women employees, with housewives being one of the key target segments for recruitment.

It said that women would be trained to service orphan policies, develop a relationship and then cross-sell to existing customers and build their network through references.

The new sales format would focus on Tier I and Tier II cities and leverage the talent pool amongst on housewives in these locations to connect with the customers and support the company?s existing distribution channels.

Apart from providing post-sales services, LPOs would also collect information related to current policies for data purification and family information and details of life events to enrich their database and help customers in reviving lapsed policies.

?The main goal of this distribution format is to target existing customers who are currently not connected to any advisor and distributor. ?We hope this sales-cum-service initiative will not only help retain our existing customers but also help us enhance relationship value through cross-sell and up-sell to them,? Who should not own the Digital Workplace? ? Mark Morrell Intranet ...

In my last post I asked ?Who should own the Digital Workplace??.? From my experience one function that I feel should not own the digital workplace is Internal Communications.

Communicators? first priority is to communicate.? Their first reaction to collaboration between employees using blogs is to increase the frequency of communications and their prominence on the intranet.

But digital workplaces are used by employees primarily to do things or find information or people, not to read communications.? They still do read communications but it is not their main purpose or first priority.

This is a dilemma that communications will need to resolve as they find a new role that continues to add value to the organisation that is more strategic.? It is NOT a good approach to seek to own the digital workplace from the view of communications being its main purpose.? It isn?t.

While communications still has a key role, increasingly it is human resources, knowledge management and business functions that are largely affected by or have a high influence on how the digital workplace is created that are increasingly involved.

A group of senior representatives who are stakeholders in the digital workplace should form something like a digital board, responsible for strategy, high-level decisions, and priorities for collaboration, communications, tools, and mobile use.

This group should have cross-organisational recognition and support that needs to be seen to be acting in their interests.? A clear strategy and prioritised action plan for the short term with owners and timescales will achieve that.

But there still needs to be a leader of the digital board whose authority is accepted.? The obvious choice would be the CEO of the organisation.? However the reality is the CEO probably won?t have enough time to focus on leading the digital board.

The next best solution is for the CEO to nominate someone or, if not possible, for there to be a senior person who is naturally seen as the ideal candidate by other digital board representatives.? The main criteria are someone whose finger is on the pulse of the organisation, is involved and aware of the key decisions being taken, and has the respect of everyone involved.

It is essential to have the right people in place who own the digital workplace strategy and future direction it will take that will benefit both the organisation and everyone working in it.

The finding of an unknown small moth by Dutch amateur moth hunter Hans Huisman in his backyard lead to the discovery that the American Oak skeletonizer (Bucculatrix ainsliella) is invading North West Europe on planted Northern Red oaks (Quercus rubra), a North American tree.

"The finding is unusual", says Erik J van Nieukerken of Naturalis Biodiversity Center (Leiden, Netherlands), "because until recently few insects attacked this North American tree outside its natural habitat." He and co-authors just published a paper on this species in the journal 'Nota Lepidopterologica', a specialist journal for the study of moths and butterflies (Lepidoptera) in Europe.

Northern Red oaks are widely planted in Europe since the introduction in the 17th century, and the tree is important as a timber tree. Only one North American aphid, specializing on this kind of oaks has previously crossed the ocean. "The Red oaks are too different from European oaks to attract many of the specialist insects that feed on those," says Van Nieukerken, "and we usually only see insects that are common on many tree species." The identity was also shown by DNA barcodes showing no difference between European and American populations.

Bucculatrix ainsliella is now common in most of the Netherlands, the northern part of Belgium and adjacent parts of Germany. The researchers expect it to be already more widely spread, and it will possibly invade most of Europe the coming years. Despite its abundant occurrence, it has nowhere lead to any visual damage, although it is known to be a pest occasionally in North America.

After the discovery was announced online and in the Dutch media in 2011, it turned out that many moth photographers and collectors in the Netherlands and Belgium had seen or collected the moth before, but were unable to identify it correctly. The earliest specimens of Bucculatrix ainsliella were found in the Netherlands as early as 1989, as was shown by overlooked specimens in the collection of Naturalis Biodiversity Center in Leiden. They were found in Amersfoort, a city close to a former NATO air base. It is very well possible that the small cocoons (with pupae) of the species, attached to any object, were transported by air. The emerging moths could immediately find their favourite food in the many Red Oaks that are planted in the Netherlands close to the airbase.

The larva of the newly discovered moth makes small leafmines in the oak leaves and later starts feeding on the leaf underside, eating the green tissue and leaving the veins intact. The effect of this feeding lead to the English name "Oak Skeletonizer". The caterpillar later makes a beautiful white ribbed cocoon that it fastens on leaves, trunks or any other item in its way. The small moth (wingspan 8 to 9 mm) can be found from April to October, and is attracted to light during the night.

When I hear the words ?fitness? and ?DVD? uttered in the same breath, late-night infomercials, over-the-top aerobics instructors and bad spandex all come to mind. According to a survey published in the American College of Sports Medicine?s Health & Fitness Journal last year, educated, certified and experienced fitness professionals took the number one spot on the list of expected worldwide fitness trends in 2012 for the fifth consecutive year. Sure, I love to dance as much as the next girl, but I?ll be honest?Tracy Anderson and her moves can be a little intimidating the first time around. Even Gwyneth Paltrow, whose brief testimonial extolling Anderson?s virtues can be found on this DVD, is willing to admit that she felt a bit lost when she first started this particular regimen. Her persistence in mastering the moves obviously paid off as Paltrow has a truly rockin? body that so belies her 40 years. Anderson recommends using this DVD in combination with either her Mat DVD or Perfect Design Series, but let?s not get ahead of ourselves?the Dance Cardio Workout features a thorough warm-up and two different routines, both of which have their own step-by-step set of instructions that break down each and every bit of fancy footwork. Unless you?re fortunate enough to have a background in dance, one run-through on this portion of the workout probably won?t be enough, but hey, that?s what the rewind button is for, right? Practice, practice, practice until you?ve really got the moves down, otherwise you?ll likely feel left in the dust when it?s time to perform (Anderson really picks up the pace during the actual routines). Once you?ve got it down, the bouncy, music video-ready choreography will have you looking and feeling like a superstar.

You probably remember Billy Blanks from (what else?) his ?90s era infomercial touting the powers of Tae Bo, a fast-paced, high-energy combination of Taekwondo and aerobics. Now that his wildly popular workouts are available on DVD, members of a whole new generation like me can kick, punch and move their feet into a total-body, calorie burning frenzy. According to an article on Livestrong.com by contributor Kristeen Moore, a fitness Web site called Fatburn.com estimates that a 160-pound individual can burn up to 764 calories per hour during a basic Tae Bo workout and up to 878 calories per hour during a more advanced version. It?s an impressive claim, but one thing?s for sure?if you move the way Blanks does for at least an hour, you?re sure to work up a decent sweat at the very least. (I?m sweating just thinking about how hard he pushes himself, not to mention his viewers.) This DVD promises a new spin on the original Tae Bo workout?it takes you through all of the original moves at a breakneck pace, and the energy in the onscreen dojo reaches a near palpable intensity on more than one occasion. Tae Bo definitely isn?t a program for the faint of heart, but Blanks? positive reinforcement and ever-present eight counts are sure to make you feel like a warrior.

Solomon Pond Mall to Open Early Black Friday - Marlborough, MA ...

Black Friday will start during what some would consider the late Thanksgiving night rather than early Black Friday morning at Solomon Pond Mall this year.

Shoppers at the mall will be able to begin their shopping as early as 12:30 a.m.

The hours are a change from last year's 4 a.m. opening and will allow shoppers to pick all the perfect gifts until 10 p.m. at night, according to a release from the owners of the Mall.?

The mall will feature greeters to help guide shoppers, as well as "Rejuvination Stations" with comfortable couches, chairs and wireless internet. Photos for children and pets with Santa will also be available that day.

Fit wrapping stations benefitting local charities will also be available at the mall.

The Auburn and Greendale Malls will also open at 12:30 a.m. on Black Friday.

NEW YORK (AP) ? Comedian Dave Attell told a packed house at the Comedy Cellar that New York after Superstorm Sandy had a familiar feel. "It was dark. Toilets were backing up. ... It was pretty much like it always was."

Another comic, Paul Mecurio, told the same crowd that he got so many calls from worried family members that he started making things up about how bad it was.

"I'm drinking my own urine to survive," he joked.

New York's comedy clubs, some of which had to shut down or go on generator power in the aftermath of the storm, dealt with a bad situation like they always have ? by turning Sandy into a running punchline.

"If they're going to do jokes on Sept. 12 about Sept. 11, then this thing isn't going to slow us down," said Vic Henley, the emcee of a show Oct. 28 at Gotham Comedy Club.

Sean Flynn, Gotham's operating manager, said comics were including the storm in their acts but had to be careful nonetheless not to make people feel worse than they already did.

"There's the old adage that tragedy plus time equals comedy. The variable is the time," he said. Still, he added: "You can't ignore the subject. That's what comedy's all about."

The Comedy Cellar, a regular stop for decades for the country's most notable comedians, was closed from Oct. 28 through Nov. 1, but reopened on Nov. 2 after a generator was brought in at a cost of several thousand dollars. Power didn't return until the next day, and the crowds came with it.

"Everyone has a bad case of cabin fever," said Valerie Scott, the club's manager.

Mecurio said he thought the joke was on him when he got a call from the Comedy Cellar saying the club was going ahead with its show even though there was no light in the West Village. He headed downtown from the Upper East Side, hitting dark streets after midtown.

"It's pitch dark," he said. "And there's a room packed with people laughing. It was so surreal. ... I'm calling it the generator show. It was a really cool thing."

"You could feel there was something special about the show," he said. "The audiences were tempered in their mood. You could tell something was up, something was in the air. I knew it was cathartic for people."

He said a woman approached him after the show to thank him, saying: "You kind of brightened my day."

Sometimes, comics used the storm to get a laugh at the expense of the crowd, like when Mark Normand looked down from the Comedy Cellar stage at a man with a thin beard.

"I like the beard," he told him. "Is that because of Sandy? You couldn't get your razor working?"

And Attell used Sandy to mock a heckler, telling him: "You must have been a load of laughs without power."

At another point, Attell looked for positives in the storm.

"There's nothing better than Doomsday sex," he said.

Mecurio said he has made a point of including the storm and the havoc it caused whenever he takes the stage.

"I feel like as a comedian in the spirit of social satire, it's what we're supposed to do," he said. "It's the elephant in the room. How do you not do it?"

Boras: Dodgers 'bought store,' Mets 'in freezer'

INDIAN WELLS, Calif. (AP) ? With baseball awash in record revenue as the signing season starts, Scott Boras compares the habits of teams to families sifting through supermarket shelves.

At the winter meetings in Dallas last year, the agent had this to say of the financially troubled Los Angeles Dodgers and New York Mets: "Normally, they're in the steaks section, and I found them in the fruits-and-nuts category a lot."

Since then, the Dodgers have been sold for $2 billion. The Mets owners have agreed to pay up to $162 million ? and likely much less ? in a deal with the trustee for Bernard Madoff's fraud victims.

So on Wednesday at the general managers' meeting, Boras said his view of the Dodgers had changed.

"I think they bought the store," he said.

And as for the Mets?

"The best you can say is that they might be in the freezer section," he explained. "But there's a lot of good, longstanding products that they can acquire there."

Slugger Josh Hamilton and pitcher Zack Greinke are among the top players in a relatively weak free-agent class that also includes outfielders B.J. Upton, Michael Bourn, Torii Hunter and Nick Swisher; first baseman Adam LaRoche; and pitchers Kyle Lohse and Rafael Soriano.

Baseball estimates revenue this year at $2.5 billion ? an increase of about $500 million. National television contracts with Fox and Turner that run from 2014-21 will double the average yearly money baseball receives to about $800 million.

And perhaps the biggest evidence of baseball's wealth is franchise values ? the Dodgers sold for $2 billion this year in a bankruptcy court auction and the lowly San Diego Padres were bought for $800 million.

"So now we've had owners that have become instant billionaires. They've made over a billion dollars by this reevaluation," Boras said. "I don't think we need Forbes anymore to tell us what franchises are worth because we had a bankruptcy court judge say this is open season, come tell me what this is all worth, and I think we got a proper accounting. Franchise values are through the roof because interestingly enough, we know what a near-top team is worth and we know what a near-bottom-value team on the market is worth by what's gone in the past three or four month. So I think there's a lot of smiling faces for people who invested in this industry in the early 2000s."

Since the Dodgers were bought last spring by a group headed by Mark Walter, Stan Kasten and Magic Johnson, they increased their payroll by about $35 million, adding infielder Hanley Ramirez, first baseman Adrian Gonzalez, closer Brandon League, pitcher Josh Beckett and outfielder Carl Crawford.

"Today compared to a year ago, we're in much better shape," general manager Ned Colletti said.

The Dodgers' payroll next year might approach or surpass that of the New York Yankees, who have topped spending every year since Los Angeles edged them by $1 million in 2001.

New York is vowing to cut its payroll by 2014 because of changes in the collective bargaining agreement that will cut the team's revenue-sharing bill if it doesn't wind up paying a luxury tax.

"Every year you're kind of given your budgets, your challenges, your needs," Yankees general manager Brian Cashman said. "You always have your restrictions, whether it's in terms of what ownership provides, the Basic Agreement allows."

It is in Boras' interest to encourage spending. His free-agent group includes Soriano and fellow closers Jose Valverde and Ryan Madson, first baseman Carlos Pena, Bourn, Lohse and pitcher Daisuke Matsuzaka.

Infielder Maicer Izturis became the first major league free agent to switch teams this offseason, agreeing to a $10 million, three-year contract with the Toronto Blue Jays. The 32-year-old hit .256 with 17 steals in 19 tries, two homers and 20 RBIs this year for the Angels.

"He's someone that's going to come in and compete for the job at second base," said Blue Jays general manager Alex Anthopoulos, who tried to acquire him last summer. "He's not guaranteed the job, but the fact that he can play all over the infield, he's just a valuable guy to have. He can do a lot of little things. He's a winning player. He can draw a walk. He can put the ball in play ? a pretty good contact rate. He can run a little bit and steal some bags. He plays very good defensively, as well, and is a good teammate, good in the clubhouse."

As the meetings conclude Friday, the deadline will pass for free agents to decide whether to accept $13.3 million qualifying offers from their former clubs. Among the eight players on the market who were given the offers, pitcher Hiroki Kuroda appears to be the only one who might accept. He is coming off his first season with the Yankees.

How Hurricane Forecasting Got So Good

As Hurricane Sandy approached New York and New Jersey last week, schools were closed, subways shut down, and low-lying coastal areas evacuated. The storm preparations were aided by what turned out to be remarkably accurate forecasts from the National Hurricane Center (NHC), which had warned residents living in Sandy's warpath as early as five days before the storm arrived. By accurately predicting the track and intensity of the storm, the forecasting may have saved lives as well as millions of dollars.

Superstorm Sandy was more complicated than a standard hurricane. Under normal circumstances, a hurricane like Sandy would have blown out to sea without giving the Northeast so much as a dirty look. But a high pressure area over Greenland steered Sandy back toward shore, where she crashed into a cold front. Despite all of that, the NHC managed to predict Sandy's track to within 50 miles and two days ahead of time. (For comparison, typical two-day hurricane forecasts are off by 100 miles on average.)

Two-day forecasts also predicted wind gusts up to 80 mph, 12 inches of rainfall, and up to 11 feet of inundation from the storm surge?predictions that almost exactly aligned with last week's storm.

"What you saw for Sandy was, I would say, the best weather forecast of a major weather system ever," says Robert Gall, who directs the Hurricane Forecast Improvement Project for the National Oceanic and Atmospheric Administration (NOAA). "I've never seen anything that accurate that far in advance."

How to Model a Hurricane


Hurricane forecasting begins with lots and lots of data. More data than weather modelers know what to do with, really. Satellites collect information about the hurricane's position, wind movement, and the atmosphere's temperature and moisture levels. Aircraft fly into the storm, collecting barometric pressure and wind intensity data. The planes drop probes that measure a variety of features, including the ocean's salinity and temperature profile as depth increases. Buoys and floats collect data about ocean currents, waves, and important interactions between the sea and the atmosphere. Meanwhile, land-based radar learns about hurricane wind fields, rain intensity, and storm movement.

Much of that data is analyzed and assimilated into a computer-generated numerical prediction model. The NHC bases its forecasts on models from all over the world?including from the Global Forecast System, the European Centre for Medium-Range Weather Forecasts, and NOAA's Geophysical Fluid Dynamics Laboratory. The models digest data differently and may use different equations to simulate how the atmospheric and oceanic conditions will change over time. Forecasters at the NHC have a pretty good idea about which models tend to perform better, and they're able to correct for the biases of each based on its past performance. They take an average of all of the models, which tends to compensate for biases in each individual model, says NHC meteorologist James Franklin.

How Sandy Forecasts Were So Accurate


"A lot of things came together this year," Gall says. "One thing we're doing is running the models at higher resolution. We're able to do that because we've got more computing power." He explains that a typical model might use a grid that consists of 1 million east?west points by 1 million north?south points by 100 vertical points. That's a lot of points (100 trillion, in fact). And at each point the models have to calculate temperature, pressure, and humidity changes at regular intervals?usually every 30 seconds. "So it takes a huge amount of computing to generate a five-to-seven day forecast," Gall says.

The NHC's ability to predict the track of a hurricane has been getting dramatically better in recent decades, thanks in part to better satellite data. In the 1970s five-day hurricane forecasts were off by 400 miles on average. In the 1990s it was down to about 200 miles. For 2010 and 2011 five-day forecasts erred by an average of just 100 miles.

It's relatively easy to forecast a hurricane's path, Franklin says, because "all hurricanes get steered by the features that surround them. When those features tend to be large, then the models do a much better job." For Sandy, the high-pressure block off Greenland, as well as the wintry front blowing in off the land, made life miserable for people in the storm's path but actually helped meteorologists to predict Sandy's track more accurately.

"The other thing that made things easier," Franklin says, "was that Sandy was not a purely tropical storm." Typical hurricanes are smaller than last week's superstorm, and the models do better with larger systems. As it moved inland, Sandy also began to take on more wintry characteristics which are easier to model, Franklin says. The reason is that hurricanes are more driven by the ocean, whereas winter storms depend on what's going on in the atmosphere, and hurricane models are stronger at modeling conditions in the atmosphere than in the oceans.

Room for Improvement


Marshall Shepherd, president of the American Meteorological Society, says it is widely recognized that U.S. weather models are not as advanced as European models. (European weather models predicted Sandy's path several days before U.S. models caught up.) Shepherd says the lag is partially because Europeans have invested in faster computers. NOAA's National Centers for Environmental Prediction already have two supercomputers dedicated to weather prediction, but Robert Gall says they need more computing power to improve hurricane prediction. In Boulder, Colo., he's been running experimental models on a supercomputer that's 10 times more powerful than NOAA's.

Having more computing power would allow meteorologists to incorporate more data into the models?currently much of the data that's collected gets thrown away to simplify things. Faster computers would also increase the models' resolution. As of now, the finest-scale hurricane models in operation have a resolution of about 2 miles, Franklin says. "That's getting to the point where we can start to depict some of the features in and around the core, [but] just barely."

A second area for improvement is in predicting the intensity of a hurricane. Intensity forecasts haven't significantly improved in several decades, because hurricane intensity is derived from its interactions with the oceans, and the ocean physics are less well understood in the models. Intensity also depends on thunderstorm activity in the core?higher resolutions are needed to replicate and predict those small-scale processes.

One promising development is the possibility of using Doppler radar observations in the core, and incorporating those observations into operational models. There again, this requires higher-resolution models and more computing power. Still, Franklin says, we might start to see this incorporated into operational models within the next few years.

For now meteorologists are better than the computers at analyzing the activity within the core and using that information to predict a hurricane's intensity, says Franklin?the models haven't learned to interpret those patterns very well. "There are some things forecasters can see that the models just don't get."

(AP) ? Greece's Parliament passed a crucial austerity bill early Thursday in vote so close that it left the coalition government reeling from dissent.

The bill, which will further slash pensions and salaries, passed 153-128 in the 300-member Parliament. It came hours after rioters rampaged outside Parliament during an 80,000-strong anti-austerity demonstration, clashing with police who responded with tear gas, stun grenades and water cannons.

Approval of the cuts and tax increases worth ?13.5 billion ($17 billion) over two years was a big step for Greek efforts to secure the next installment of its international rescue loans and stave off imminent bankruptcy.

The country's international creditors have demanded that the bill and the 2013 budget, due to be voted on Sunday, pass before they consider releasing an already delayed ?31.5 billion installment from Greece's ?240 billion bailout. Without it, Prime Minister Antonis Samaras says Greece will run out of money on Nov. 16.

"Greece made a big decisive and optimistic step today. A step toward recovery," Samaras said, adding that he was "very happy" with the result.

Development and growth for the country, which faces a sixth year of a deep recession in 2013, will come "only with a lot of work, with coordinated action, with investments," he said.

But the close vote was a major political blow to the three-party coalition government, which holds a total of 176 seats in Parliament. The result shows support for continued austerity three years into Greece's financial crisis is dwindling fast.

"The government now has very little margin to take measures like this again," said Dimitris Mardas, associate professor of economics at the University of Thessaloniki. "But unless it takes various obvious actions like limiting the black economy, addressing tax evasion and improving the country's investment framework, we may end up needing new measures. And then things will be very difficult."

Straight after the vote, two of the three coalition parties ? Samaras' conservatives and former finance minister Evangelos Venizelos' socialists ? expelled a total of seven dissenting deputies from their ranks.

Lawmakers from the third, the small Democratic Left, mostly abstained from the vote in accordance with their party's line. Leader Fotis Kouvelis had said he could not back labor reforms included in the bill.

During hours of acrimonious debate in Parliament, Samaras acknowledged that some of the measures in the bill were unfair, but insisted there were vital to avoid bankruptcy and Greece being forced out of the euro and back to its old currency, the drachma.

"This (bill) will finally rid the country of drachmophobia," he said.

"Many of these measures are fair and should have been taken years ago, without anyone asking us to," Samaras said. "Others are unfair ? cutting wages and salaries ? and there is no point in dressing this up as something else." But, he said, the alternative was bankruptcy that would trigger financial chaos as the country would likely have to leave the 17-country euro bloc.

The measures are for next year and 2014, and include new, deep pension cuts and tax hikes, a two-year increase in the retirement age to 67, and laws that will make it easier to fire and transfer civil servants who are currently guaranteed jobs for life.

The reforms aim to lower public debts but will in the process also hurt the economy, which is set to enter a sixth year of recession with unemployment at a record 25 percent.

"You are throwing people onto to the street, people who need a few more years till they get their pensions," said Panagiotis Lafazanis of the main opposition Syriza, or Radical Left, party. "What will happen to them? Will they starve?"

Ahead of the vote, tens of thousands of protesters braved torrential rain to shout anti-austerity slogans. The rally eventually turned violent outside Parliament, with hundreds of rioters hurling gasoline bombs and chunks of marble at police. Clouds of tear gas rose from central Syntagma Square as the police fought back.
